The spine/hip movement into the ball on the? down swing is a common cause for anyone with a weak lower back. Also try using something as simple as a tee, blade of grass or anything to mark the "boundary" for the toe then you cannot miss the sweet spot.

I think your absolutely right when you spoke about balance being the main factor on this issue. My balance isn't great and I'm sure when I hit a shank? my feet tend to move forward towards the ball.
